OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by hazards: a) On or about 10/30/18 - Pallet Room, an employee was caught between the platens while attempting to dislodge stuck cardboard in a horizontal baler. The employee stepped onto the baler to gain a height advantage and therefore did not follow the training and safety procedures as instructed by the company when the accident in question occurred.
