29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about November 14, 2018, during the installation of roofing material at a job site located at 775 Clayton Ln, Aurora, Ohio 44202; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of heights approximately 10.9 feet above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about November 14, 2018, during the installation of roofing material at a job site located at 775 Clayton Ln, Aurora, Ohio 44202; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 10.9 feet by utilizing a fall protection system.   The ANTHONY ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1268165, citation number 01, item number 02 and was affirmed as a final order on (12/12/2017), with respect to a workplace located at 4418 8TH ST NW, Canton, Ohio 44708.
