29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about May 26, 2020, during the installation of roofing material at the job site located at 6020 Canterbury St NE, Canton, Ohio 44708; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about May 26, 2020 at the job site located at 6020 Canterbury St NE, Canton, Ohio 44708;  during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 26, 2020, during the installation of roofing material at a job site located at 6020 Canterbury St NE, Canton, Ohio 44708; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 9 feet by utilizing a fall protection system.     The ANTHONY ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1268165, citation number 01, item number 02 and was affirmed as a final order on (12/12/2017), with respect to a workplace located at 4418 8TH ST NW, Canton, Ohio 44708.     The ANTHONY ROOFING LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Duty to have fall protection), which was contained in OSHA inspection number 1360076, citation number 02, item number 01 and was affirmed as a final order on (3/27/2019), with respect to a workplace located at 775 Clayton Ln, Aurora, Ohio 44202.
