29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that unused section of the blade was adequately guarded while employees operated the K&K vertical band saw # 155 (Serial # 203), located in the maintenance room, to cut corners of boxes. During the band saw operation, the employees' hands were located in a close proximity to the unguarded blade, exposing them to amputation hazards. B) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the Aktiebolaget Strands drill press # 156 (Type S53L; Serial # 33180) located in the maintenance room. Employees were exposed to struck-by, laceration, and amputation hazards. C) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the MSC Industrial 711 VS drilling machine # 200 (Model # 9512419; Serial # 3405065) located in the tool room. Employees were exposed to struck-by, laceration, and amputation hazards. D) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, the rotating chuck of the Carlton drilling machine # 201 (4ft. arm on 113col.; Serial # 1A-1503 DRC 16854) located in the tool room. Employees were exposed to struck-by, laceration, and amputation hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: A) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that the left side of the Baldor bench grinder # 90 (Catalog # 8123W, Serial # 3524M), located in the maintenance room, was equipped with work rest that was adjusted to 1/8 inch away from the abrasive wheels. Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheels breakage. B) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer did not ensure that the right side of the Baldor bench grinder # 84 (Catalog # 8123W), located next to the upset press # 5, was equipped with work rest that was adjusted to 1/8 inch away from the abrasive wheels. Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheels breakage.

Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheels breakage. B) Edgerton Forge, Inc., located at 257 East Morrison Street, Edgerton, OH 43517: On or about November 15, 2018, the employer failed to ensure that the left and the right sides of the Baldor bench grinder # 87 (Catalog # 8123W; Serial # Y0602027289), located next to the upset press # 7, were equipped with tongue guards that were adjusted to ¼ inch away from the abrasive wheels.
