29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures:    (a)  On or about November 15, 2018, and at times prior employees were potentially exposed to hazardous energy because the procedures to isolate machines from hazardous energy did not include specific steps to test the machines and verify that the energy control measures were effective.  The incomplete procedures included, but were not limited to those for the following machines:   seamers in the cutting line; bevelers in the flat line (Max 80 CNC Bavelloni and Elettromeccanica Bouone); Billco washers at the cutting line and in the insulated glass area; and power grinder #13 in the polishing area. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  (a)  On or about November 15, 2018, and at times prior employees were potentially exposed to hazardous energy because the employer did not inspect, nor certify, the sufficiency of the Glaz-Tech Industries lockout/tagout procedures at least annually with the purpose of ensuring that the procedure is sufficient, and that the authorized personnel are following it properly.    Abatement Note:  The periodic inspection shall be performed by an authorized employee other than the one(s) utilizing the energy control procedure being inspected (29 CFR 1910.147(c)(6)(I)[A]).  The employer shall certify that the periodic inspections have been performed by documenting the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection (29 CFR 1910.147(c)(6)(ii)). 29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    (a)  On or about November 15, 2018, and at times prior, the authorized employee was potentially exposed to hazardous energy because he was not trained on the Glaz-Tech Hazardous Energy Control program and the procedures that he was expected to utilize.    Abatement Note:  In addition to each authorized employee, affected employees shall be instructed in the purpose and use of the energy control procedures (29 CFR 1910.147(c)(7)(i)[B]); and other employees whose work operations are in the area where energy control procedures may be utilized, shall be instructed about the procedure(s) and the prohibition of attempting to restart or reenergize machines or equipment which are locked out or tagged out (29 CFR 1910.147(c)(7)(i)[C]). 29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed:    (a)  On or about November 15, 2018, and at times prior, employees were exposed to electrical hazards where rigid conduit was not properly connected and the electrical wires inside were exposed:    1) The pull box for the wires that powered the outlet to which the retractable extension cord was connected in the cutting area, was disconnected from the outlet where it was supposed to be attached at the knockout, and the cover of the box was loose.  2) The metal clad conduit connected to the shape beveler, was broken where the electrical power is brought into the machine (note:  this is a wet area).
