29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   a) Lot #106,On or about 11/19/2018, and at times prior, the employer exposed employees to struck-by hazards, in that the use of head protection was not enforced while the employees were setting trusses on top of the second story wooden stud walls.

29 CFR 1926.502(b)(11): When guardrail systems are used at holes, they were not erected on all unprotected sides or edges of the hole.   a) Lot #106, On or about 11/19/2018, and at times prior, the employer exposed employees to fall hazards of up to eleven feet, five inches, in that a complete guardrail system was not installed on the second floor around the top of the open stairway.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) Lot #106, On or about 11/19/2018, and at times prior, the employer exposed employees to fall hazards of up to eleven feet, five inches, in that the stairs to the second floor were not equipped with a stair rail system along the unprotected edges.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction located on Lot #106, On or about 11/19/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that protective eyewear was not provided to employees engaged in wooden truss installation. Employees operating these tools were exposed to struck-by hazards which included plastic collated fragments, wood debris and 3-inch fasteners.   Elmer Sanchez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1306209, citation number 1, item number 1 and was affirmed as a final order on 06/18/2018, with respect to a workplace located at Lot #75 Portsmouth Court Marietta, Ga. 30064.

Powder Springs, Ga. 30127, On or about 11/19/2018, and at times prior, the employer exposed employees to fall hazards of up to twenty-one feet, five inches, in that the use of personal fall arrest systems were not enforced while the employees were setting trusses on top of the second story wooden stud walls.   Elmer Sanchez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1306209, citation number 1, item number 2 and was affirmed as a final order on 06/18/2018, with respect to a workplace located at Lot #75 Portsmouth Court Marietta, Ga. 30064.
