29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location: 1250 Osgood Street, North Andover, MA On or about November 9, 2018, employees were not adequately instructed in the recognition and avoidance of unsafe conditions and did not take the necessary precautions to prevent entry into the area of the rotating superstructure of the Komatsu excavator.
