29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a) On or about 11/19/18, on a roofing jobsite, at a house located at 816 Fisher Rd., West Seneca, NY: A Ladder Jack Scaffold was erected on the rear deck of the house. The scaffold had two support ladders, and the brackets that supported the aluminum pik were on the outside (the side away from the house) of the ladder. An employee used the scaffold to access & exit the roof. The employee stepped down from the roof onto the pik, climbed down and over the bracket on the west support ladder. A separate access ladder to the pik was not provided. NO ABATEMENT CERTIFICATION REQUIRED
