29 CFR 1910.303(g)(2)(i)(D): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by elevation of 2.44 m (8.0 ft) or more above the floor or other working surface: On or about November 21, 2018, employees working on or accessing the store mezzanine were exposed electrical shock hazards from unprotected light fixtures that were less than 8 feet from the mezzanine floor.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: On or about November 21, 2018, employees who work on the store mezzanine were exposed to fluorescent light tubes that were not guarded against physical damaged and their height from the mezzanine floor was approximate six feet, exposing employees to struck-by hazards.
