29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: (a) On or about November 14, 2018, and at times prior, the employer did not ensure that portable fire extinguishers used by employees were mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. Employees were exposed to fire hazards and were subject to injuries in that: 1. A dry-chemical portable fire extinguisher located on the South side of the outdoor yard was not mounted and identified. 2. A dry-chemical portable fire extinguisher located on the South West corner of the outdoor yard was not readily accessible. Access to the fire extinguisher was blocked by snow and debris. 3. A dry-chemical portable fire extinguisher located by the forklift refueling station was not mounted and had not been inspected since 2015. 4. A dry-chemical portable fire extinguisher located in the warehouse was not mounted, and not identified. 5. A dry-chemical portable fire extinguisher located on the entrance to the break room was not identified. 6. A dry-chemical portable fire extinguisher located inside the break room was not mounted. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
