29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: a) Park 1 Parking Garage, City of Billings: On November 27, 2018 employees were exposed to fall hazards while doing leading edge work from a fall height of approximately 58 feet to the concrete side walk below.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder: (a) Park 1 Parking Garage, City of Billings: On November 27, 2018 employees were exposed to fall hazards while using a ladder to access the roof of approximately 15 feet from the roof to the concrete parking level that was not extended 3 foot above the surface.
