29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as required in Table D-2 of this standard: (a)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees accessing the north mezzanine in the shop area were protected from fall hazards in that the stairs used to access the mezzanine, having more than 4 risers, did not have an appropriate guardrail/handrail. This exposed employees to fall hazards. (b)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees accessing the south mezzanine in the shop area were protected from fall hazards in that the stairs used to access the mezzanine, having more than 4 risers, did not have an appropriate guardrail/handrail. 29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures: (a)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2019, the employer did not have a written respiratory protection program with worksite specific procedures, as specified in subparagraphs (c)(1)(i) through (ix) of this section. The program was not established and implemented whenever respirators were required by the employer in that, the employer required the use of a 3M half-face, negative pressure, air-purifying respirator during paint spraying operations. This hazard exposed the employee to the lack of knowledge of proper respirator use. Abatement Note: The employer shall include in the program the following provisions of this section, as applicable: (1) Procedures for selecting respirators for use in the workplace; (2) Medical evaluations of employees required to use respirators; (3) Fit testing procedures for tight-fitting respirators; (4) Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations; (5) Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators; (6) Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators; (7) Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations; (8) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance; and (9) Procedures for regularly evaluating the effectiveness of the program.

This condition exposed employees to laceration hazards. (b)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the KAAST HBS A230 horizontal bandsaw were protected from laceration hazards in that, the unused portion of the blade was not appropriately guarded. This condition exposed employees to laceration hazards. (c)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Hendley and Whittmore Co. roll former #18099 were protected from an in-going nip point associated with gearing on the side of the roller near the operator's station, was not appropriately guarded. This condition exposed employees to caught-in hazards. (d)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Fabrica Macchien Bergamol horizontal band saw were protected from lacerations in that, the unused portion of the blade was not appropriately guarded. This condition exposed employees to laceration hazards.

(a)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Edwards 50 Ton Jaws IV Iron Worker were protected from amputations in that, the punch point of operation on the iron worker was not appropriately guarded. This condition exposed employees to amputation hazards. (b) Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Fabricator Talona hydraulic press were protected from amputation hazards in that, the point of operation was not appropriately guarded. This condition exposed employees to amputation hazards. (c)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Accupress hydraulic press were protected from hazards in that, the point of operation was not appropriately guarded. This condition exposed employees to amputation hazards. (d)Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Hendley and Whittemore roll bender were protected from crushing hazards in that, the point of operation of the roll former was not appropriately guarded. This condition exposed employees to crushing hazards. (e) Farm Aid Equipment, Inc. at 40 East 1st Street, Corsica, SD 57328: On or about and at times prior to November 27, 2018, the employer did not ensure employees required to perform work on the Piranha P70 Ironworker were protected from amputations in that, the point of operation of the punch portion was not appropriately guarded. This condition exposed employees to amputation hazards.
