29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    (a) Front Side of the Two-story Multi-unit House at 21 West Areba Avenue, Hershey, Pennsylvania - Employees used a ladder jack scaffold to access the roof, which had an eave height of 16 feet.  These employees did not use any fall protection while on the platform of the ladder jack scaffolding, exposing the employees to the hazard of falling to the ground below, on or about November 7, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) Front Side of the Two-story Multi-unit House at 21 West Areba Avenue, Hershey, Pennsylvania - Employees worked from roof bracket scaffolding to tear off transite roofing shingles on the roof, which had an eave height of 16 feet.  These employees did not use any fall protection, exposing them to the hazard of falling to the ground below, on or about November 7, 2018.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) Front Side of the Two-story Multi-unit House at 21 West Areba Avenue, Hershey, Pennsylvania - Employees worked on the roof tearing off transite roofing shingles.  The roof had a 10 in 12 slope and had an eave height of 16 feet.  The employees did not use any fall protection, exposing them to the hazard of falling to the ground below, on or about November 7, 2018.    H.W. Wilson Roofing Co., Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1073291, citation number 1, item number 1 and was affirmed as a final order on October 19, 2015, with respect to a workplace located at 2209 Market Street, Harrisburg, Pennsylvania.    Abatement certification and documentation is required within 10 days after the abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
