29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform)was not installed so that the space between adjacent units and the space between the platform and the uprights was no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:  On or about November 29, 2018, employees were exposed to a fall hazard of greater than 10 feet while applying stucco to a residential construction without adequate planking on the scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  On or about November 29, 2018, employees were exposed to a fall hazard greater than 10 feet, while applying stucco to a residential construction from a scaffold platform without guardrails or other fall protection.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces were used as a means of access.  On or about November 29, 2018, employees were exposed to a fall hazard greater than 2 feet while applying stucco to a residential construction without a ladder for access.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not  protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer did not develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:  On or about November 29, 2018, an employee was exposed to a fall hazard greater than 6 feet while applying stucco to a residential construction without fall protection.

29 CFR 1926.1053(b)(4): Ladders were not used only for the purpose for which they were designed:  On or about November 29, 2018, an employee was exposed to a fall hazard while applying stucco from a Gorilla ladder, Model MPX22W, that was in a closed position, leaning on the roof, and supported by a 2" x 4" piece of lumber.
