On or about November 29, 2018, employees performing sewer and water installation operations were exposed to struck by hazards when employees worked on two different levels when the employer failed to ensure hard hats were used. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. On or about November 29, 2018, employees performing sewer and water installation at a commercial property in a 8.6 foot deep trench were exposed to cave-in hazards when the employer failed to ensure a means of egress was provided.
