29 CFR  1926.200(g)(2):Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA.  On or about June 23, 2021 employees exposed to vehicular traffic were not provided with traffic control devices, exposing employees to struck-by hazards.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.1153(c)(1):For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section:  On or about June 23, 2021 the employer did not implement engineering controls, work practices and or respiratory protection for an employee dry cutting concrete, exposing the employee to the hazards associated with Silica.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about June 23, 2021 employees working around a back hoe were not provided with hard hats, exposing employees to struck-by hazards.  Wescon Underground Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1363088, citation number 1, item number 1 and was affirmed as a final order on February 26, 2019, with respect to a workplace located at 820 S. Wright St. Naperville, IL 60540.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about June 23, 2021 an employee working in a 5.2 foot deep trench was not provided with a safe means of access, exposing the employee to the hazards associated with no safe access to a trench.  Wescon Underground, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 1363088.015, citation number 1, item number 2 and was affirmed as a final order on February 26, 2019, with respect to a workplace located at 820 S. Wright St. Naperville, IL 60540.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
