29 CFR 1910.110(f)(2)(i): Containers in storage shall be located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons.    (a) Loading Dock: LP-gas cylinders were located inside at the overhead doorway entrance to the plant. The LP-gas cylinders were free standing and not protected from physical damage from powered industrial trucks that used the entrance on or about November 30, 2018.

The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace.
