29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) The employer did not report an in-patient hospitalization that was the result of a work related incident to OSHA within twenty-four (24) hours for an injury that occurred on November 11, 2018. The injury was reported on November 13, 2018. Menards, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1904.39(a)(2) which was contained in OSHA Inspection Number 1056836, Citation number 1, Item number 2 and was affirmed as a final order on June 18, 2015 with respect to the workplace located at 2221 N. Stevens Street, Rhinelander, Wisconsin 54501.
