29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a) Gloucester County Library roof, 389 Wolfert Station Road, Mullica Hill, NJ: Employees working on a section of the roof a minimum of approximately 9 feet above ground level, having an approximate 5/12 pitch, without any fall protection, were exposed to trip hazards such as: old broken shingles were laying around on the roof, removed flashing and down spouts piled in middle of the roof, on or about 11/08/2019. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Gloucester County Library roof, 389 Wolfert Station Road: Several employees in the process of reroofing were working at different levels on the roof, between approximately 9 feet at the eaves and 31 feet at the peaks of the roof and on the ground below the eaves of the roof without any head protection, on or about 11/08/2018. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
