On or about December 3, 2018, employees were exposed to fall hazard greater than 10-feet due to scaffold not being fully planked and not using any type of fall protection when working off a tubular scaffold.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about December 3, 2018, the employer did not ensure that each employee working from a scaffold was trained by a qualified person. Employees were exposed to fall hazards from an elevation exceeding 10 feet.
