29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On Friday November 30, 2018 at approximately 5:00am, an employee was operating a ride on pallet jack when he suffered an amputation tip of the 1st digit on the right foot. The amputation was not reported within 24 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on 3 December, 2018 at approximately 2:00pm.
