29 CFR 1904.29(b)(3):  The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:     Soudan Metals Company, Inc. dba Steel Fab & Finish, Inc., Chicago, Illinois - On or about December 6, 2018, the employer failed to enter correctly the following workplace injury for the calendar year 2018 on the OSHA form 300 and 301 or their equivalent forms within seven (7) calendar days of receiving information that a recordable injury or illness had occurred.    a) On or about September 27, 2018, a recordable injury occurred when an employee carrying a stack of metal pans lost control of the pans and was struck in the forearm by one of the metal edges.  The employee sustained a laceration to the forearm, resulting in 70 days away from work (as of December 6, 2018), however the employer did not record the injury on an OSHA 301 Incident Report.  Additionally, not all portions of the OSHA Form 300 were completed, including the information requested in Column A (enter case number), Column E (enter where the event occurred), Column F (enter what object directly injured the employee), Column H (check if there were days away from work, and Column M1 (check for injury).
