Warehouse - On or about September 17, 2025, the employer did not ensure that employees operating sheet metal feed conveyors were trained in the purpose and use of the company's energy control procedures. A machine operator sustained fractures to the right humerus, forearm, and elbow when their right glove became caught in a measuring wheel as the employee inspected a 40 x 76-inch steel sheet for defects.
