29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: (a) On or about November 30, 2018, employees were exposed to explosion and fire hazards created by explosive dust which was allowed to accumulate in excess of 1/32 inch on structural members, floors, on and around equipment, and areas around dust collectors and ductwork.

29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials, or that could develop in areas with an accumulation of dust: (a) On or about December 12, 2018, employees were exposed to explosion and fire hazards in that cutting and welding operations were permitted by Idaho Pacific Colorado Corp. on the Idaho Steel Products Baghouse, model #96TB-BHI-81:S6 dust collector intake vent that was not adequately cleaned, resulting in a fire.

29 CFR 1910.252(a)(2)(xiii): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment on its property, in accordance with 29 CFR 1910.252 (a)(2)(xiii)(A) through 29 CFR 1910.252(a)(2)(xiii)(D)(a): (a) On or about December 12, 2018, employees were exposed to explosion and fire hazards in that cutting and welding operations were permitted by Idaho Pacific Colorado Corp. on the Idaho Steel Products Baghouse, model #96TB-BHI-81:S6 dust collector intake vent that contained flammable material, resulting in a fire. Idaho Pacific Colorado Corp. failed to train designated personnel on the hazards associated with cutting and/or welding on an enclosed piece of equipment that contained flammable material.
