29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed.  Building 3030 warehouse; on or about 27 November 2018 and times prior thereto, two instances were noted were exit routes were not kept free and unobstructed:       a. Building 3030, walk-in freezer; the employer did not ensure that the emergency release mechanism was readily accessible. A Jamison Mark IV walk-in freezer, had pallets of frozen products stored in front of the emergency escape mechanism for door number two.        b. Building 3030, three designated exit routes were not substantially level and stairs or a suitable ramp was not provided:             (1) Food service office area, the designated exit route was not substantially level and stairs or a suitable ramp was not provided. Surface was covered with snow and has 46-inch change in elevation from the loading ramp.            (2) Shipping and packing area, the designated exit route was not substantially level and stairs or a suitable ramp was not provided. Surface was covered with snow and has 46-inch change in elevation from the loading ramp.            (3) Stock readiness area, the designated exit route was not substantially level and stairs or a suitable ramp was not provided. Surface was covered with snow and has 46-inch change in elevation from the loading ramp.   This condition exposed employees to a hazardous indoor work environment and fire hazards.

Building 3490, welding shop; on or about 27 November 2018 and times prior thereto; the employer did not ensure that the Delta ShopMaster Drill Press, model DP350, serial number 027274W3054, was securely mounted to prevent movement. This condition exposed employees to a struck by hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  Building 3490, welding shop; on or about 27 November 2018 and times prior thereto; a 50-foot heavy duty blue extension cord had visible damage to the outer protective jack adjacent to the cord plug. This condition exposed employees to an electrical shock and electrical burn hazard.

Building 2116, Alert Holding Area, bay area; on or about 27 November 2018 and times prior thereto; eleven metal warehouse pallet storage racks were not secure to the wall or floor. Pallet racks are located in a seismically active zone. This condition exposed employees to a struck by hazard.

Building 3030, warehouse ISSA area EG/EH; on or about 27 November 2018 and times prior thereto; the employer did not ensure that a Ladder Industries nine-step steel mobile ladder stand was capable of supporting at least four times their maximum intended load. No load rating was indicated. This condition exposed employees to a fall hazard.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit".  Building 3490, wood shop; on or about 27 November 2018 and times prior thereto; the employer did not ensure that exit signs were posted along the designated exit route. Two instances were noted where exit sings were not posted.       a. No exit signs were posted along the exit route from the wood shop leading to the fabrication room.       b. No exit signs were posted along the exit route from the wood shop to the welding bay.  This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.
