29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained and kept free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  DMWR Building 1185, Birch Hill Ski and Snowboard Area, west egress: on or about 28 November 2018, a loose sheet of plywood board measuring approximately 80x40 inches was used as a regular walking surface covering a construction trench. This condition exposed employees to a fall hazard.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems.  DMWR Building 1185, Birch Hill Ski and Snowboard Area, compressor room, valve pit: on or about 28 November 2018, the entry point to the rail compressor pit was not guarded, exposing employees to fall hazards of approximately 4 feet. This condition exposed employees to fall hazards.

A construction pit running parallel to the building existed within the exit path impeding a timely egress. This condition exposed employees to a fire hazard.

This condition exposed employees to delays in exiting the building, smoke inhalation, fire, and tripping hazards.

This exit is designated as the primary exit. No emergency exit lighting available along this route.       b) The door way leading to the outdoor exit route was not marked. This exit is designated as the alternate exit route. No emergency exit lighting available along this route.  This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

This condition exposed employees to an electrical shock hazard.      b)  Building 3702, Nugget Lanes Bowling Center, center lane access area: on or about 28 November 2018, an energized ProNautic digital charging platform, model 2420P had visible damage to the outer protective jack adjacent to the cord plug. This condition exposed employees to an electrical shock and electrical burn hazard.      c)  Ski Area, DMWR Building 1185, west-side building exterior: on or about 28 November 2018, an energized 25 foot orange extension power cord used to heat snow groomers had visible damage to the outer protective jacket in the form of cracks and strain relief. This condition exposed employees to an electrical shock hazard.      d)  Outdoor Recreation, Building 4050, workshop bench: on or about 28 November 2018, an energized 25 foot extension power cord had visible burn damage to the outer protective jacket. This condition exposed employees to an electrical shock hazard.

Golf Course, Building 2095, adjacent to the western exit door:  on or about 28 November 2018, the employer did not ensure that Honeywell manual operated fire pull station was clear of obstructions. Fire pull station had miscellaneous items stored in front of the pull station impeding immediate access. This condition exposed employees to smoke inhalation and fire hazards.
