This most recently occurred at 3606 Yost Ave., Hastings, Nebraska 68901 on December 3, 2018. The employer failed to ensure that point of operation guarding was used on the Press 58 during setup. An employee reached into the unguarded machine to retrieve a bolt. The machine actuated, causing an amputation of the employees finger.
