Throughout the facility  The employer is failing to protect employees from electrical shock and amputation hazards.  The employer is failing to conduct inspections/evaluations of the lock-out/tag-out program.  Failure to conduct these inspections precludes the employer from identifying potential lapses in the program including, but not limited to, lack of more than one authorized employee in the facility.

29 CFR 1910.147(c)(7)(i):     The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. Not all employees, who are exposed to unexpected energization, have received this training. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    Throughout the facility  The employer is failing to protect employees from the hazards of machines by not properly guarding them from amputation hazards on equipment including, but not limited to, press brakes and drill presses.  Employees are exposed to amputation hazards due to the lack of guarding of these machines.

Shipping Area in the facility located at 3606 Yost Ave., Hasting, NE  The employer is failing to protect employees from laceration hazards by not properly ensuring the equipment is positioned to ensure the head of the saw returns to its starting position.  The radial arm saw is not equipped with a counter weight system nor is it positioned to ensure the head of the saw returns to its starting position.

29 CFR 1910.252(b)(2)(iii):     Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles.    Main production area in the facility  The employer is failing to provide adequate welding screens in the welding area to protect employees from ultraviolet rays during welding operations.
