Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On or about 12/12/2018, Tuepker Brothers Construction, LTD was installing roofing materials on a building located at 1306 Sunview Drive in OFallon, MO. Two employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while working on a 4/12 sloped roof approximately ten feet above the ground.
