OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a caught-between hazard: (a) On or about December 7, 2018 employees at the site were exposed to a caught-between hazard when working beneath and around a load suspended by a riser clamp. Among feasible and acceptable means of abatement would include: 1. Use of approved lifting clamps designed with the rated capacity. 2. Comply with ASME B30.20-2010 Below-the-Hook Lifting Devices. Use a lifting device designed and manufactured in accordance with Chapter 20-1.
