29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative. At the time of the inspection, it was determined that a lockout device had not been installed on the new conveyor in the leg quarters department. An employee received a finger tip amputation on 11/18/2018 when he placed his hand in a hazardous area and another employee energized the conveyor.

29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof. At the time of the inspection, it was determined that on 11/18/2018 employees were not removed from the machine area prior to the testing of the new wing conveyor. When the new conveyor was energized an employee had his hand in the machine and received a fingertip amputation.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points. At the time of the inspection, the south end of the wing tip conveyor had a metal plate welded which created a ingoing nip point. The conveyor travelled to the north with an opening starting at 1/2 inch on the east to 3/4 inch on the west.
