29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. 1) Jobsite: Employer did not provide training that allowed employees to recognize, evaluate and mitigate hazards associated with scaffolds. This condition was noted on/about 29 NOV 2018.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization: a) Jobsite: On or about 29 NOV 2018, an employee was admitted to the hospital as an in-patient for treatment of a work-related injury upon falling from a scaffold approximately 13 feet. The employer did not notify OSHA of the injury. OSHA learned of the in-patient hospitalization on 10 DEC 2018.
