Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 26 to 29 feet.  b) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA - Exterior Scaffold Northwest- On or about June 9, 2023, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 6 feet.  c) Jobsite located at or near 821-25 North 13th Street, Philadelphia PA - Exterior Scaffold Northeast- On or about June 9, 2023, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. GTO Construction, INC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1511579, Citation Number 2, Item Number 1 and was affirmed as a final order on 7/29/2021, with respect to a workplace located at Messina Way and San Vencenzo Drive, Philadelphia, PA 19145.  GTO Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1366683, Citation Number 1, Item Number 1 and was affirmed as a final order on 10/04/2019, with respect to a workplace located at 3200 Napoli Way, Philadelphia, PA 19146.
