29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Facility, 650 Trumbull Drive, Pittsburgh, PA: On or about December 18, 2018 the Empire sandblaster was not turned off and the energy sources controlled during a maintenance procedure. The air source was not completely closed and/or powered off and locked out. During the maintenance procedure pressure was built up and released as the employee was changing the desiccant air dryer filter. The employee sustained a head injury when he was struck by a flying part.
