29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  On or about December 17, 2018, in the north side of the shop, employees were exposed to electrical shock hazard from a flexible cord used to power a Hypertherm Power Max welder (P/N 083234; S/N 65-049049) with the outer insulation pulled from the plug end of the cord exposing the inner wiring.

29 CFR 1910.305(g)(2)(iii): Identification, splices, and terminations.  Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  On or about December 17, 2018, in the north side of the shop, employees were exposed to electrical shock hazard from a flexible cord used to power a Hypertherm Power Max welder (P/N 083234; S/N 65-049049) with the outer insulation pulled from the plug end of the cord exposing the inner wiring and the welder was not removed from service.

29 CFR 1910.303(b)(7)(iv): Electric equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  On or about January 25, 2019, at the north end of the shop, employees were exposed to electrical fire and shock hazard from:  a) Main service electrical distribution panel case that was severely dented;  b) Main service electrical distribution panel cover was unsecured and missing bolts; and  c) Subpanel adjacent to the Main panel with an unsecured panel cover and missing bolts.
