29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection: (a) First Floor - On or about December 18, 2018, employees were observed using a Craftsman air compressor plugged into a Ridge 5 Outlet Extension Cord Hub that was plugged into an outlet that did not have a ground fault circuit interrupter (GFCI). In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. (a) Second Floor - On or about December 13, 2018, the workers were working adjacent to a window that had a ledge 18 inches above the floor, while disposing of material being removed as a part of demolition activities without using fall protection and exposed to a fall hazard of approximately a 16 feet. No abatement documentation or certification required for this item.
