29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    (a)  First Floor - On or about December 18, 2018, employees were observed using a Craftsman air compressor plugged into a Ridge 5 Outlet Extension Cord Hub that was plugged into an outlet that did not have a ground fault circuit interrupter (GFCI).    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    (a)   First Floor - On or about December 18, 2018, employees were observed using a Craftsman Air compressor to power hand tools, in which strain relief was not provided.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    (a)  Second Floor - On or about December 13, 2018, the workers were working adjacent to a window that had a ledge 18 inches above the floor, while disposing of material being removed as a part of demolition activities without using fall protection and exposed to a fall hazard of approximately a 16 feet.    No abatement documentation or certification required for this item.
