29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components are operationally intact: a) On or about December 17, 2018, at the facility, where employees had not inspected the work area of Screw Conveyor 515 Bin 11 to ensure that all machine components, such as conveyor covers, were operationally intact prior to the removal of lockout devices and restoration of energy. Employees were exposed to caught-in hazards during maintenance activities such as removing obstructions from the screw auger.
