29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Employee(s) were exposed to fall hazards. Employee(s) engaged in painting activities were exposed to fall hazards while working at height in the aerial lift man basket. Employee(s) were not provided with training to recognize, evaluate, and control the hazards, such as fall hazards, associated with working at height in the man basket.
