OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to pressure vessel overpressure hazards associated with a jacketed steam kettle which was not installed and maintained to prevent or minimize risk for employee injury during an overpressure event: a. 1090 Springfield Rd., Union, NJ - Small Kitchen Employees engaged in the manufacture of food products were exposed to pressure vessel overpressure hazards associated with a Groen, DN RA-150-T316 Steam Jacketed Kettle . The steam kettle suffered an overpressure that lead to vessel failure and projection of separated components of the equipment. After the failure, the kettle bowl bounced off the floor and was imbedded in the ceiling. Employees sustained injuries after being struck by the blast from "steam jacket" which had separated from the kettle bowl. Violation occurred on or about December 20, 2018

29 CFR 1910.219(f)(2): Hand-operated gears. Paragraph (f)(1) of this section does not apply to hand-operated gears used only to adjust machine parts and which do not continue to move after hand power is removed. However, the guarding of these gears is highly recommended: a. 1090 Springfield Rd., Union, NJ - Small Kitchen The employer did not ensure that hand operated gears were protected to prevent employee injuries. Employees engaged in the manufacture of food products manually operated a gear and sprocket system to empty the contents of the kettles that included, but was not limited to Groen, DN RA-150-T316 Steam Jacketed Kettle(s), that was not protected. Violation occurred on or about December 20, 2018
