29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      a. On or about December 22, 2018 the ladder being used by employees did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder, exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on December 22, 2018, when employees are working on a residential roof at 93 Wynnfield Drive, Palm Coast, FL 32164 without fall protection and are exposed to falls of approximately 11 feet to the ground below.     Turnkey Construction Planners, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 958012, citation number 1, item number 1, and was affirmed as a final order on 03/26/2014, with respect to a workplace located at 854 Lorenza Place, Rockledge, FL, 32956.     Turnkey Construction Planners, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13) which was contained in OSHA inspection number 1164124, citation number 1, item number 1 and was affirmed as a final order on 3/28/2017, with respect to a workplace located at 11356 SW Patterson Street, Port St Lucie, FL 34953.
