The employer failed to establish a regular housekeeping schedule that would have reduced the accumulation of dust in the building. The lack of housekeeping allowed dust (milled feed) to accumulate on walking-working surfaces in the entire work area, milled feed is a combustible dust.

On or about December 26, 2018 after an industrial hygiene analysis and evaluation was performed at the Old Mill: Employees were exposed to fire and explosion hazards. The employer failed to properly classify the area and provide intrinsically safe electrical equipment throughout the entire work area. Electrical equipment including, but not limited to, overhead lights, circuit breaker panels, disconnect switches and outlets, in Class II, Division I location were not intrinsically safe, or approved for a Class II, Division I, or safe for a Class II, Division I. Some equipment was compromised or not properly designed and would allow for the intrusion of dust into and on equipment that contained live electrical or mechanical parts.
