29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Authorized and affected employees throughout the facility were exposed to electrical, struck-by and caught-between hazards in that the employer did not assess all equipment to develop a control of hazardous energy written program consisting of energy control procedures, and employee training to identify potential hazards of unexpected energization or start-up or release of stored energy including electric, hydraulic, pneumatic and gravity.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Employees engaged in maintenance activities throughout the facility were exposed to electrical, struck-by and caught-between hazards in that machine specific energy control procedures had not been developed and implemented. Equipment included but was not limited to the Extrusion Line 1 to Line 10 machines.    29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Employees engaged in machining activities at the Line 1 and Line 2 Extrusion area were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized for the Roll Stand #1, 2, and #3. The rotating rolls were not guarded to provide protection from rotating parts.    b) Employees engaged in maintenance activities in the maintenance shop inside the main plant building were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized for the Enco 9-Speed Drill Press Model # 126-2235. The rotating drill chucks were not guarded to provide protection from flying chips and debris.    c) Employees engaged in maintenance activities in the maintenance shop inside the main plant building were exposed to hazards in that the unused portions of the blade on the Emerson 7" Metal Cutting Band Saw, Serial #8164.P0013 were not properly guarded, leaving employees exposed to a 2.5-inches opening on the right side of the blade, and 7.5-inches on the left side of the blade with the guard was installed 1-inch above the blade.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:  Employees engaged in maintenance activities in the maintenance shop inside the main plant building were exposed to injury in the event of wheel failure in that the distance between the left side adjustable tongue guard and the abrasive wheel periphery measured 0.5 inch for the 8" Bench Grinder.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  A work related incident which resulted in an employee hospitalization and finger amputation was not reported to OSHA within 24 hours. The incident occurred on or about 12/28/2018 at 11:00 pm and was not reported to OSHA until 01/03/2019 at 3:07 pm.  29 CFR 1903.19(c)(2): Abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition:  In the main plant building, identifying markings were absent from the control levers to indicate the actions the levers performed for the Toyota forklift #18, Serial #73108, two out of the three indicator levers were worn, faded, and not legible.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
