29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.  On or about December 31, 2018, an employee was exposed to machine hazards associated with a moving band saw blade when they engaged in servicing and maintenance activities on the L2 Biro Band Saw.  The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to allowing employees to clean the Biro Band Saw on Line 2.  The machine was not shut down or turned off prior to engaging in this servicing and maintenance activity.  b. Employees routinely would clean pizza crust trimmings from the L2 Biro Band Saw, during normal operation without controlling the hazardous movement of the blade, and the guard did not protect employees from the moving blade where the guard was cut at a 45 degree angle near the band saw table.  The machine was not shut down or turned off prior to engaging in this servicing and maintenance activity.    As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
