29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy:    The employer is failing to protect employees from the release of stored energy when performing maintenance on machinery.  This was most recently observed at 1216 East 19th St, Columbus, Nebraska on January 3, 2019 when employees were provided LOTO procedures for the Hem Saw that did not include steps to remove potential stored air pressure. The LOTO procedures identify a 100 PSI air source but no steps to safely remove the stored air pressure.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    The employer is failing to protect employees from electrical and amputation hazards associated with performing maintenance on equipment with written specific procedures that have not been reviewed at least annually.  This was most recently observed at 1216 East 19th St, Columbus, Nebraska on January 3, 2019 when the employer had employees performing maintenance on equipment that require the implementation of lockout and/or tag out as means of preventing unintentional energizing without having conducted the annual periodic review of the written specific procedures.  Instances of exposure include but are not limited to:       a. Employees using written control procedures dated 2011 as the guidelines for locking out the hazardous energy for the Hurco Mill       b. Employees using written control procedures created on an unknown date as the guidelines for locking out the hazardous energy for the Hem Saw.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycles:    The employer is failing to protect employees from amputation hazards while they are operating cutting machinery.  This was most recently observed at 1216 East 19th St, Columbus, Nebraska on January 3rd, 2019 when an employee was observed operating a horizontal bandsaw and the guard did not cover the exposed blade.  There was 2"of exposed blade while the employee was operating the saw.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected.    The employer is exposing employees to struck by hazards when operating pneumatic air equipment. This was most recently documented at 1216 East 19th St, Columbus Nebraska on January 3, 2019 when a compressed air hose was observed with a hose clamp attaching it to a drill press.
