OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by or caught-in hazards: Location: Separator Room, North Roto-Screen Lockout procedures that had been developed and documented by the employer were not utilized by two employees consistently during their shift, exposing them to struck-by hazards and caught-in hazards. One feasible means of abatement for this hazard would include ensuring that employees utilize and comply with all elements of the lockout program.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) Separating Room: South Roto-Screen - A shaft from a sixty to 1 (60:1) gear reduction box measuring approximately one (1) inch diameter by two and three-quarter (2 3/4) inches in length with an unused keyway was not guarded exposing employees to caught-in and struck-by hazards. (a) Separating Room: North Roto-Screen - A shaft from a sixty to 1 (60:1) gear reduction box measuring approximately one (1) inch diameter by two and three-quarter (2 3/4) inches in length with an approximately quarter (1/4) inch keyway was not guarded exposing employees to caught-in and struck-by hazards.
