This most recently occurred on January 8, 2019 on the jobsite located at Lot 180 in the Pebblebrooke 2 Subdivision, S. 169th St, Omaha, NE 68046.  Five employees were installing new roofing materials on the roof of a newly constructed house and not utilizing any form of fall protection.  The house has a six in twelve steep sloped pitch and ground to eve height is approximately eleven feet.

Five employees are using an extension ladder to access and egress the roof where the side rails were not extended to at least 3 feet above the landing surface which is about eleven feet above the ground.
