29 CFR 1910.37(a)(2): Exit routes were not arranged so that employees did not travel toward a high hazard area:  (a) Building S34, Room 112-1. Emergency evacuation of employees to the outdoor exit results in travel with increasing proximity to the dust collector. The emergency exit at the wood shop was approximately 6.7 feet away from the Donaldson Torit dust collector, model UMA 250, serial number 10534223-L1-1. A dust collector bulk sample is explosive and the Kst test result is 3.6 bar*meter/second. These conditions were noted on or about 03/28/2019. Employees are exposed to possible caught-in and struck-by hazards while operating the American Tool Inc. Lathe, S/N 79557, in that the rotating chuck was not properly guarded. These conditions were noted on or about 01/09/2019.    (b) Room 111-A. Employees are exposed to possible caught-in and struck-by hazards while operating the ACRA Machinery Inc. Lathe, S/N 2161407817, in that the rotating chuck was not properly guarded. These conditions were noted on or about 01/09/2019.   (c) Room 111-A. Employees are exposed to possible caught-in and struck-by hazards while operating the General Manufacturing Co, 15" Drilling Machine S/N P7675, in that the rotating shaft and point of operation was not guarded. These conditions were noted on or about 01/09/2019.  (d) Room 111-A. Employees are exposed to possible caught-in and struck-by hazards while operating the Cleereman Dill Press S/N 62431, in that the rotating shaft and point of operation was not guarded. These conditions were noted on or about 01/09/2019.  (e) Welding Shop: Employees are exposed to possible caught-in and struck-by hazards while operating the Wilton drill press S/N 15870,in that the rotating shaft and point of operation was not guarded. 29 CFR 1910.334(a)(2)(ii): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a) Room 118B: The outer jacket of a flexible cord on a relocatable power tap used to power a microwave and coffee pot was damaged and pulling away from the plug, exposing employees to potential shock and fire hazards.
