Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) Warehouse, LPG container storage: On January 9, 2019, and at times prior, the employer exposed employees to burn hazards while removing and replacing LPG containers on forklifts, in that proper skin protection was not provided.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) Warehouse: On January 9, 2019, and at times prior, the employer exposed employees to struck-by hazards in that Forklift (Daewoo Model G15S-2 Serial Number DG-00057) had a frayed hydraulic hose located at the mast but was not taken out of service.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables may not be used as a substitute for the fixed wiring of a structure. a) Production Area, M31 Tug Tight Operator Station: On or about April 19, 2017, and at times prior, the employer exposed employees to electrical shock and fire hazards while utilizing an electrical bag sealer that was connected to an outlet powered with an extension cord, which was tied to the building structure.
