In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. Production area, various machine wiring enclosures: On or about September 1, 2022, and times prior, the employer exposed employees to electrical shock hazards in that electrical conductor were not installed in an approved enclosure as to prevent accidental contact. b. Production floor (TB3008) On or about September 1, 2022, and times prior, the employer exposed employees to electrical shock hazards in that electrical receptacle/outlet were found to be missing the cover plate.
