29 CFR 1910.178(f)(2): The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA No. 58-1969, Storage and Handling of Liquefied Petroleum Gases. Section 6.21.3.4(B): Fuel containers shall be mounted to prevent jarring loose and slipping or rotating, and the fastening shall be designed and constructed to withstand, without permanent visible deformation, static loading in any direction equal to four times the weight of the container filled with fuel:  a: On or about January 10. 2019, outside the bay doors of the main shop area, the employer failed to ensure that removable LP-Gas containers were securely mounted on Yale fork lift S#B449027 to prevent displacement.  b: On or about January 10. 2019, outside the bay doors of the main shop area, the employer failed to ensure that removable LP-Gas containers were securely mounted on Nissan fork lift S#95132 to prevent displacement.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  a) On or about January 10, 2019, in the shop, the employer failed to prevent employees from using multiple electrical extension cords that had strain relief damage.
