Section 6.21.3.4(B): Fuel containers shall be mounted to prevent jarring loose and slipping or rotating, and the fastening shall be designed and constructed to withstand, without permanent visible deformation, static loading in any direction equal to four times the weight of the container filled with fuel: a: On or about January 10. 2019, outside the bay doors of the main shop area, the employer failed to ensure that removable LP-Gas containers were securely mounted on Yale fork lift S#B449027 to prevent displacement. b: On or about January 10. 2019, outside the bay doors of the main shop area, the employer failed to ensure that removable LP-Gas containers were securely mounted on Nissan fork lift S#95132 to prevent displacement.
